Requires three-year commitment that includes pre- and post-season evaluations from all participating coaches.

Application process
Phone for information and to have application packet sent by mail. Application packet may also be downloaded from website any time and submitted by regular mail. Each application must include a minimum order for 60 helmets/goggles (each team is designated five pieces). There is a deadline of December of each year to ensure equipment is received by the begining of season.
