Eligibility

-Participants must be at least 60 years of age and reside in Lake County. -Household income from all resources for one is less than $26,973 and $36,482 for a household of two.

Required documents

Proof of income. Valid Driver's License if serving as a proxy for a homebound senior.

Application process

Fill out the application online: aging.ohio.gov/sfmnp A card will be mailed to the client if they choose that option during registration. Otherwise a digital code will be sent to the client's phone. Seniors who need assistance with shopping may list a proxy to shop on their behalf. A person can only be proxy for up to 2 individuals. The name of the proxy and their driver's license number are needed at the time of application.
