MEDICAID HEALTH INSURANCE - AGES 19-64; THROUGH 138% FPL; NOT ON MEDICARE | CUYAHOGA COUNTY JOB AND FAMILY SERVICES
Provides public insurance to cover medical services that may include doctor visits, some prescription drugs, hospital care, home care, long-term care, medical equipment, transportation, mental health, vision, dental, substance abuse services and other services. Most clients will have to choose and enroll in (will have their care provided by) a Managed Care Plan (MCP). When it is time to renew, enrollees will receive a packet, including information regarding Medicaid status and forms for re-certifying coverage.

Application process
Clients may begin the application process, renew, and/or update their case information by calling the Eligibility Contact Center: M-F: 8:00am-1:00pm (due to high call volume, caller may be given the option to receive a call back - if they choose this option they should get a call within 1-2 business days). May also apply, renew, and/or update case information through website anytime (client must have an email address to do this). No interview is needed. Agency can look up most supporting documents through an electronic records search - the client will be sent a checklist for any missing supporting documents. Once an eligibility determination is made, a notice of action (NOA) is mailed to the applicant. Applications are available and may be dropped off at any Neighborhood Family Service Center (NFSC) M-F: 9:00am-4:00pm. Free scanning is available at the Virgil Brown Building M-F: 9:00am-4:00pm (drop box at this location is available 24 hours a day). Completed application may also be faxed (number is on the application). Free faxing is available at branches of the Cleveland, Cuyahoga, and Shaker Heights Libraries during their normal hours of business. MCP-eligible clients must select and enroll in an MCP, or will be auto enrolled randomly (plan can be changed with reason, or during open enrollment). Those with general questions about Medicaid, Medicaid Managed Care Plans, or who simply need to change their address/contact information can call the Medicaid Customer Service Hotline M-F: 7:00am-8:00pm; Sa: 8:00am-5:00pm.
Fee
No fee to apply. Once receiving coverage, individual MCPs may charge co-payments for some services.
Eligibility
Adults age 19-64 who are not currently receiving Medicare. Must live in Cuyahoga County and be a U.S. citizen, legal resident for five years, or legally residing non citizen with refugee or parolee immigration status. Financial eligibility is set at 138% FPL and is based solely on the taxable income of applicant and their dependents, not household income. For example, since employment, alimony and unemployment compensation are taxable they would be included as income, but child support, Supplemental Security Income (SSI), and workers' compensation are not taxable so they are not included. Individuals who are eligible for at least $1 of SSI are automatically eligible for Medicaid.

Operates several Neighborhood Family Service Centers which provide cash benefits (such as Ohio Works First welfare benefits), food assistance (food stamps), medical coverage, employment/training assistance, child care vouchers, and supportive services to Cuyahoga County residents who meet eligibility requirements. Clients are assigned to a Service Center according to their zip code. Spanish-speaking staff are available.
