Serves Kentucky employees that have experienced a work-related injury or disease.

Other documents (Employer reports must contain: -employer’s name, location, and type of business -employee’s name, age, sex, wages, and occupation -date and time the injury occurred -the nature of the injury and its cause)

Application process

Report injury or disease to supervisor or another designated person as soon as possible after the event. Employers must report the injury or disease to its workers’ compensation insurance carrier within three days of receiving notification. If the injury or disease requires an employee to miss one day or more from work, the insurance carrier must report the injury or disease to the Department of Workers’ Claims.
