Health insurance card, if applicable. Those planning to request financial assistance services will need to provide proof of family income.
Appointments are generally available M-F: 8:00am-5:00pm. Some evening and weekend appointments may be available.

Third-party payment accepted, including Medicaid and Medicare. Those who have no health insurance must have household income at or below 400% of federal poverty guidelines to apply for financial help. Financial Counselors will screen uninsured for Marketplace insurance, Medicaid, or hospital charity eligibility.

Offers care for urology problems, including prostate disorders, kidney disease; testicular, bladder, prostate, adrenal and kidney cancer; bladder control; sexual dysfunction/impotence; male infertility;chronic urinary tract infections and obstructions; pediatric urology; kidney and pancreas transplantation; kidney stones; and laparoscopic surgery.
