Application process
For glasses call Randy. Only helps individuals one time with eye exam and/or glasses. For glasses to be donate, donation boxes are located at all library locations.

No fees.
Eligibility
Residents of Ontario and persons living west of Main Street in Mansfield. If they are East of Main Street in Mansfield they need to call the Madison Lions Club. May also help persons in the Shelby or Lexington area.
