Application process
Intake is conducted upon first appointment, usually requiring 1.5 hours. Medical insurance and income information is recommended for this appointment to determine fees, if any, for services. Contact our office for an appointment.
Fees
Medical insurance, Medicare and Medicaid accepted. Self-pay is determined by sliding fee scale. Crime victim counseling may be available at no charge.
