- Case management services are available to residents of Portage and Geauga counties. - Serves young adults ages 18 through 24. Must be a victim of crime (such as domestic violence or sexual abuse/assault), or at risk of becoming a victim of crime. Definition for "at-risk" population is broad and includes young adults who are homeless (no safe place to stay at night and/or "couch surfing"), aging out of the foster system, have a history of experiencing abuse or neglect while in foster care, or thrown out of parents' home.

Description

Provides case management services to young adults. Clients will be paired with a trained case manager who will help develop a personalized case plan to foster independence. Participants will learn skills such as financial planning, job readiness, study skills, time management, goal-setting, problem-solving, and critical thinking. Residents of Portage County will be provided with help in accessing existing housing programs.

Provides a broad continuum of community-based services to individuals and families in Northeast Ohio. Services include hot meals, emergency groceries, shelters for the homeless, emergency housing assistance, services for children, support for victims of domestic violence, veteran’s assistance, community and senior outreach, and full clinical services including counseling, case management, psychiatry and addiction services. Services provided vary by county.
