Individuals/families with disconnection notice, need new service/transfer of service or reconnect service, have less than 25% fuel oil or propane, wood, coal or wood pellets.

Required documents

Birth certificate or Social Security card, proof of income, and recent bill for gas/electric/primary heating source, if applicable.
