Provides person-centered support to older adults and individuals with disabilities as they work through programmatic and eligibility determination processes. Includes: - Exploring the needs expressed by an individual during completion of the Long-Term Services and Supports Questionnaire (LTSSQ). - Identifying potential services and supports that may help meet those needs. - Providing linkage to services and supports identified. - Supporting individuals as they determine which path will best meet their needs. - Determining the level of support individuals may need to take next steps. - Providing appropriate level as of on-going support to individuals based on their needs.

DIRECTION HOME AKRON CANTON AREA AGENCY ON AGING AND DISABILITIES

Plans, funds, and coordinates services for older persons and persons with disabilities in Portage, Stark, Summit, and Wayne counties to live with dignity and choices in their homes and communities for as long as possible. Administers PASSPORT, MyCare Ohio, The Assisted Living Waiver, The Ohio Home Care Waiver, Care Coordination, Elder Rights, Family Caregiver Support Program, Nursing Home Placement Assistance Program, and other services designed to help older adults and individuals with functional limitations maintain independence. Provides funds for services administered by other community agencies including client finding, specialized information and referral, transportation, home-delivered meals, medical treatment, personal care, respite care, safety monitoring, health assessments, congregate meals, recreational activities, and adult day care.
