Application process
Anyone can refer a person to the Gatekeeper Program. The following information is needed:
- Name.
- Date of Birth (if known).
- Address.
- Phone number (if known).
- Presenting problem or concerns.
- Referral Source Name, Contact Information and Agency.

Proactively works with local agencies to identify older people 60 years of age and over and adults with known or suspected Alzheimer's disease or a related dementia, who are living alone, have an intellectual developmental disability and/or have caregivers in needs of support.
Once a referral is received a nurse and social worker will make a home visit to assess the situation, begin linking to community resources and provide case management support.
