OHIO HOME CARE WAIVER / PROGRAM | CUYAHOGA COUNTY JOB AND FAMILY SERVICES

Provides an integrated program of home care services designed to address the specific home care needs of those under the age of 60 needing nursing and/or daily living services. Two types of benefits packages are available: Home Health Services and Waiver Benefits. Home Health Services provide nursing, daily living, and skilled therapy. Waiver Benefits provide the same services but may also include: home delivered meals, emergency response systems, home modifications, adult day health services, out-of-home respite care, social work, and care coordination. (Upon turning 60, waiver recipients are automatically transferred onto the Transition II [Carve-out] Waiver, which is only available to these individuals, and cannot be applied for independently)

Application process

Phone for information. Interested persons should call to be placed on a waiting list. This program requires assessment at both the county and state levels, and time on the waiting list may be lengthy - applicants with questions about the progress of their application should contact their caseworker for assistance.

Fee

Paid through Medicaid, but there may also be some patient liability.

Eligibility

Serves Cuyahoga County residents. Home Health Services are for individuals of any age in Cuyahoga County who need nursing services every day because of unstable medical conditions. Waiver Benefits are for those age 59 or younger whose medical condition and/or functional status would qualify them for Medicaid coverage in a nursing home or hospital.

Operates several Neighborhood Family Service Centers which provide cash benefits (such as Ohio Works First welfare benefits), food assistance (food stamps), medical coverage, employment/training assistance, child care vouchers, and supportive services to Cuyahoga County residents who meet eligibility requirements. Clients are assigned to a Service Center according to their zip code. Spanish-speaking staff are available.
