Families or individuals with serious mental illness and/or addiction disorders and experiencing homelessness (living on the streets, in a shelter, a parked car, abandoned building, or place not meant for habitation)
Referrals are accepted from the community waiting list held by the TLCHB. To be placed on the waiting list you must have a SPDAT completed by trained representatives from your mental health agency or one of the Lucas County homeless shelters.

Description
Provides supportive housing to individuals with serious mental illness and/or addiction disorders. Program participants are provided supportive services including home visits, peer support, and skill building to persons with a mental illness, reducing psychiatric hospitalization and the risk of homelessness.
