Individual must reside in Ashland County and have a cancer diagnosis.
Required documents
Completed application, diagnosis sheet signed by physician and completed travel log sheet.

Application process
Walk in or call for more information. Clients must submit their receipts and/or travel log sheet by the 10th of each given month to receive a check by the 20th of that month.

Clients are provided a monthly check for reimbursement for mileage, prescriptions, medical and nutritional supplies related to their cancer treatment and recovery.
