Provides educational programming to help older adults manage type 2 diabetes, arthritis, hypertension, lung disease, and other illnesses and conditions. The goal is to slow the progression of illness as long as possible without the use of more intensive health care services. Teaches relaxation techniques, healthy eating, managing sleep and fatigue, managing medications and monitoring blood sugar, exercise, and communicating effectively with healthcare providers.
