Open to Medicaid recipients who have Humana as their State Medicaid Managed Care Organization (MCO) and

  • are currently pregnant or have recently given birth (within the past 3 months)
  • complete a comprehensive assessment and at least 1 follow-up call with a HumanaBeginnings Care Manager
