Provides a variety of services that enable individuals to remain independent and cared for at home. Included are planning for families with a member who requires an increased/increasing level of care; recommendations and referrals to address problem areas; counseling to deal with life change, loss, anxiety and depression; physical, occupational, or speech therapy; skilled home nursing; personal emergency response systems that alert a remote agency when someone has fallen and requires assistance. Service includes caregiver training.

Application process

Phone M-F: 8:00am-5:00pm , and staff will determine if an in-home assessment appointment should be scheduled. Evaluations are done by social workers.

Fee

Fixed fee, payment plans are available. Third party payment accepted, including Medicare and Medicaid. Approved for reimbursement through PASSPORT. Subsidies to holocaust survivors are based on financial need. Personal emergency response systems have a separate installation fee as well as a monthly fee and may not be covered by insurances for those younger than 60.

Eligibility

Some services are directed to older adults, aged 60 and over, some to disabled adults (18 and over). Serves Eastern Cuyahoga County and western parts of Lake County, and Eastern Geauga Counties.
