Children 4-18 years old and their families. Children must meet one or more of the following criteria: poor school performance, poor parent-child interactions, poor peer relations skills, history of family violence and abuse and symptoms that interfere with normal functioning.
For initial assessment please bring insurance or Medicaid card, Child's Social Security card, proof of monthly or annual income, Ohio Driver License/State ID and custody papers (if applicable).
