Application process
Application required. Applications are accepted online at https://gms.kidneyfund.org/login Applications are reviewed on a first-come, first-served basis, based upon time of submission. Applications are typically processed within 10-14 business days.

Eligibility
Applicants must: -live in and receive dialysis treatment for end-stage renal disease (ESRD), -be receiving health insurance coverage, -have a household income at or below 500% of the federal poverty level, and -have a total amount of liquid assets (excluding retirement accounts) at or below $30,000 Note: Patients already receiving assistance from any of the following programs are automatically eligible: Low-Income Home Energy Assistance Program (LIHEAP), Temporary Assistance for Needy Families (TANF), HUD housing assistance, or Supplemental Nutritional Assistance Program (SNAP).
