Case/care Management For Individuals With Mental Illness/emotional Disabilities | Dcch Center For Children And Families
Open to residents of Boone, Campbell, Grant, and Kenton County ages 4-64 who:
- Receive Kentucky Medicaid insurance
- Have a qualifying mental health issue/concern
- Are in need of additional educational, emotional, social, housing and/or employment support

Description
Provides assistance with obtaining and maintaining connections to resources within the community through an intensive case management format. Participants will meet with a case manager several times a month for a period of 3-6 months to assess their needs, develop a care plan, and work with community providers to eliminate barriers to wellbeing.
