Parole - Assertive Community Assessment (p-act Team) | Recovery Resources

Provides intensive case management for those on parole or post-release control who have a severe mental illness. Offers the services of social workers, psychiatric nurses, a psychiatrist, and a parole officer. Maintains frequent interaction with client and monitors their medications, basic needs, and housing and employment situations, and helps them find needed treatment and services.

Application process

Must be referred by the Ohio Prison System, Ohio Adult Parole Authority, or a Case Worker from a social service agency. Phone for information during business hours.

Adults 18 and older who are on parole or post-release control and have a severe mental illness. Serves Cuyahoga County residents. Client is enrolled until parole sentence is completed.
