Phone for information and intake M-F: 9:00am-4:00pm. ALL INDIVIDUALS MUST HAVE A CASE WORKER. If you do not have a case worker, you can call for your mental health screen and a case worker can be assigned to you. Mental health assessment must be completed, once completed, an appointment for an assessment will be made. Once the assessment is complete, if appropriate the client will be enrolled for services.
Fee
Accepts payment through Medicaid (however, client does not have to already be enrolled in Medicaid for initial intake).
Eligibility
Cuyahoga County residents. Serves those age 4 and older at all sites except The East Health and Wellness Center which only serves Adults age 18 and older.
