Residents of Cuyahoga County, age 60 or older. Must live alone in a private home or apartment and have an income at or below 200% of the federal poverty level.

Description

Provides a senior wellness program designed to support the safety, emotional well-being, and social connection of older adults. Services include daily automated check-in calls to monitor well-being and safety, along with monthly one-on-one calls to discuss individual needs and provide referrals to community resources. Optional counseling sessions are available for additional mental health or personal support. Ongoing contact helps reduce isolation and promotes a sense of connection and community for participating seniors.
