Eligibility

Ages 18 and over with opiate use disorders. Participation requires at least three sober supports for opiate user.

Photo ID: accepts a variety insurance card. For sliding scale: proof of residency and income: accepts a variety of proofs.

Application process

Assessments are required. Phone M-Th: 8:00am-5:00pm and F: 8:00am-4:30pm to schedule. Do not leave messages in the general mailbox. During the day, press the Operator key or listen to the list of departmental voice mailboxes to leave a message with the Medication Department. Assessments are generally scheduled within three days.
