- Serves residents of Portage County households. - Must have at least one (1) minor child. - Must have income at or below 200% of the Federal Poverty Guidelines. Specific items are provided based on the age of the child: - Birth to 3 months: Crib/mattress, child safety kit, and newborn clothing pack. - 3 to 6 months: Stroller. - 6 to 9 months: High chair, bibs, plates, utensils, and Sippy cups; - 12 to 18 months: Potty chair, two (2) cases of pull-ups, baby wipes, and age appropriate enrichment toys. - 22 to 28 months: Two (2) cases of pull ups, baby wipes, books, music, and age appropriate enrichment toys. - 32 to 38 months: Toddler bed and mattress. - Families may apply at any point in the child's development up to age 38 months. - This program can serve multiple children in the same household, and families may utilize this program each time their child/children reaches one of the milestone age ranges listed above.
- Completed application form. - Proof of address. - Proof of financial need (all income which has been received by any member of the household during the previous 30 days will be considered. Gross income, both earned and unearned, will be counted.) - If pregnant, need a medical statement verifying pregnancy. - In addition, if the cost of the documented need exceeds the amount of PRC that is to be issued, the applicant must provide verification that they have paid the amount in excess of the PRC payment, prior to receiving payment. - Those who need cribs will need verification of the urgency of the situation as well as an invoice or other document from the store listing each item separately and the price for each item. - If needed, Agency will help clients pay for any required documentation that the client does not already have.
