Provides hot meal to eligible individuals. Diabetic and low-salt diets can be accommodated with prior arrangement.
Offers home delivered meals, both hot and frozen, for people who are homebound, unable to prepare meals, or have no one to help. The need may be temporary or long-term. Dietary accommodations include kosher, diabetic-friendly, gluten-free, renal, and cardiac therapeutic meals. Case managers work closely with clients to address nutritional needs.
Free meals are available Mondays through Fridays after school for youth at select Library locations. Participants must eat their meals at the library.
Provide temporary shelter for single women and women with children who are homeless. Length of stay is 30 days with goal of moving to stable housing. Assistance with finding permanent housing and other resources.

Program provides in-home alternatives to nursing home care for eligible individuals. Services include: a personal care plan to meet the individual's needs at home by arranging an appropriate mix of services to supplement care provided by family members and friends. Eligible services include: adaptive and assistive equipment, assistance in the home with health activities such as taking medications appropriately and other health-related activities, with case/care management, adult day care, chore service, home medical equipment and supplies, emergency response systems, home delivered meals, homemaker services, independent living assistance, minor home modifications, nutrition consultation, personal care services, medical and non-medical transportation. Program also administers the Pre-admission Review process that determines the level of care for persons who want to enter Medicaid-certified nursing homes and periodically offers vendors training on dealing with elderly/disabled individuals.
