Provides van transportation to shopping, church, medical appointments and other locations. Clients that are non-ambulatory will be transported via a regulated ADA (Americans with Disabilities Act) vehicles or through an ambulance service.

Transportation for Newton Falls residents age 60 or over or disabled for in-town grocery shopping, banking or medical appointments in the surrounding area.

Provides transportation to and from private dwellings in Washington County to medical appointments. Transportation is available during normal business hours.

Provides transportation to center's functions, grocery shopping, and the Dayton Mall.

Provides transportation services or mileage reimbursement for Medicaid recipients for non-emergency medical transportation for miles traveled to a Medicaid covered service when the managed care plan provider cannot provide service.

Provides gas vouchers, rides, or bus fare for recipients who have reached their ride limit through their Medicaid HMO.

Finds ways to meet members' needs through organized social and educational events, volunteer assistance, referral to service providers, and opportunities for engagement within the Clintonville-Beechwold Neighborhood. This program includes weekly social opportunities, with outings suggested by the members. This program also includes a wide range of volunteer services including transportaiton to medical appointments, the grocery store or social outings, snow removal, technology assistance, and more.

A door-to-door service that offers pick-up and drop-off within Pickaway County. Reservations may be made up to four weeks in advance. All busses are wheelchair accessible.

Provides resource and service programs to address transportation issues. Programs include: * Free in County Transportation: Provides one free transportation benefit a month. Includes round trip with 2 stops within Wooster city limits, provided by a contracted transportation provider. All trips require 48 hour notice. Total household income at or below 125% of the Federal Poverty Guidelines. * City of Wooster: Provides reduced rate, subsidized transportation passes to travel within Wooster city limits. Passes include one-way cab pass, one-way work/education pass, and orange/white 3rd shift and Sunday shift work pass. Total household income at or below 200% of the Federal Poverty Guidelines. Must live within Wooster city limits and be 62 or older, have a disability, or be a veteran or active member of military. * Wooster Transit: Transit passes available for purchase at Community Action-Wayne office. * Mobility Management: Travel Trainers provide transportation information and resource coordination for seniors, people with disabilities, and low-income individuals. * Out of County Medical Transportation: Provides free out-of-county medical transportation to seniors, people with disabilities, and low income (at or below 200% of the Federal Poverty Guidelines) residents. Up to 6 round trips per year provided. Wheelchair transportation available, and riders allowed to bring one travel companion. Call or email transportation department to enroll. Must provide 72 hour notice.

Transportation services are available to people 55 years and older with a demonstrated need due to physical limitations that prevent the use of public transportation. Unlimited medical trips, local and long distance. Medical riders 60 and over must contact O'Neill Senior Center as primary, Senior Wheels as back-up. For riders 55-59, Senior Wheels is primary. One trip each month for personal errands like grocery shopping, pharmacy or enrichment. All transportation services are provided by RSVP volunteers using their own vehicle, volunteering their time. Local grants enable RSVP to provide mileage reimbursement to the drivers.

Provides public fixed-route bus services within Northern Kentucky and downtown Cincinnati, including local, express, and airport routes.

Provides non-emergency medical transportation up to three times per month. Will transport north to Toledo and south to Findlay.

Provides a public transit system for the Miami Valley. Buses are equipped with wheelchair lifts, and many features such as automatic voice annunciators that announce bus stops, times and major intersections buses offer a kneeling feature which allows the operator to lower the front door to curb height to assist passengers with boarding.

Provides an electric bike, helmet, and lock. Participants must participate in a bike safety training program (provided by the agency).

Individuals 60 and over may qualify for transportation to medical appointments or other appointments that support health and wellness.

Interpreting, captioning, case management, advocacy, life skills, assistive listening devices, sign language, classes for public and family members of the Deaf, Youth Enrichment and Recreation Club, school and summer programs, college internships.

Provides door-to-door bus rides to and/or from Final Fridays at the Brickyard.

Mobility Management coordinates transportation services and - Educates consumers about transportation resources and programming. - Coordinates trips and needs between agencies - Educates communities about transportation options - Provides assistance with program eligibility. Also works with public, non-profits, and all other sources of transportation providers and services the elderly, disabled, and general public.

Provides a Medicaid-funded program that offers home health care and services coordination. Services include personal care, homemaking, emergency response system, home-delivered meals, adult day services, medical transportation, counseling services, medical equipment and supplies, and long-term care planning.

Provides transportation to the elderly and disabled for necessary trips such as grocery shopping and doctors' appointments, within the Village and a limited area outside of the Village.

Provides transportation to medical appointments and to businessess around the Berea, Middleburg Heights and Strongsville area. Medical trips receive priority. All other trips are based on the availability of the vehicles.
