Self-directed home care support program. Eligible veterans are approved for annual budget that can be used to hire/purchase services to help avoid nursing home placement. Types of services provided include: * Personal care tasks * Meals or other home delivery services * Respite care * Care coordination * Goods and services to improve home safety * Case management provided by the Area Agency on Aging * Provided through collaboration with the Louise Stokes Cleveland VA Medical Center.

Assists individuals with developmental disabilities in developing the skills that are necessary to stand up for themselves; obtain the information, opportunities, respect, and recognition to which they are entitled and the services for which they are eligible, and help them integrate into and become valued members of their communities.
