Delivers primary health care in the home through a hospital-based interdisciplinary team to homebound (often bedridden) eligible veterans, whose caregivers are willing and able to assist in their care. Provides medical and nursing care, rehabilitation services, dietetic consultation, psychosocial assessment, educational services, medications, and equipment.

Provides skilled home health care services to residents of Montgomery County. Services include: nursing; physical, occupational, and speech therapy; nutrition; enterostomal therapy; intravenous (IV) therapy; social work; and home health aides.

Licensed midwives and physicians provide childbirth education and health care services for pregnant women during pregnancy and delivery. Offers pregnancy tests, pap tests, birth control, and STD testing. Also offers a fitness program to encourage bone density and nutrition services. Provides clients supportive services including social work and nutrition services.
