The Health Center is a patient-centered medical home offering preventive and primary care to patients of all ages. Medical services will include, but are not limited to: treatment of illness such as ear aches, colds, sore throats, sinus infections and management of chronic disease such as diabetes; physical exams for work, school and sports; well-child exams; immunizations for children and adults; and women’s health including pregnancy tests and Pap tests. Dental services include cleanings, fillings, sealants, extractions, x-rays, fluoride treatments, oral health exams and oral cancer screenings. Other clinical services include substance abuse treatment and counseling services. Health Center staff will also connect patients with other community services that can help them, including tobacco cessation classes and nutrition counseling. Other services include: illness prevention, pharmacy, on-site social worker, diabetes education, high blood pressure treatment, lab services, and x-rays.

Blood Pressure screening is available upon request. No physician referral is necessary, however, they are often performed at the request of a physician to assist in monitoring the patient's hypertension. Individuals are welcome to come in at any time and have their blood pressure checked. Cholesterol - ALT/AST Screening is provided by appointment. A 12-hour fasting period prior to testing is recommended to ensure accuracy of your testing. Cholesterol screening measures the following: - Total Cholesterol - HDL (good cholesterol) - LDL (bad cholesterol) - Triglycerides - ALT/AST (liver function) - Glucose. Our nursing staff will explain your test results and provide you with American Heart Association approved lists of foods to help bring your cholesterol numbers in line with current AMA recommendations. We will fax or mail your cholesterol results to your physician at your request. Comprehensive Metabolic Panel (CMP) testing measures the following: NA+, K+, CL-, CA - electrolytes or minerals that regulate cell function and internal water balance TCO2 - bicarbonate that keeps the blood from becoming too acidic or too basic GLU - glucose BUN / CRE - kidney function ALP, ALT / AST, TBIL - liver function ALB & TP - proteins in the blood, kidney and liver function Our nursing staff will explain your test results. We will fax or mail your CMP results to your physician at your request.

This program provides self-management educational tools for individuals with Type II Diabetes or those at risk of developing diabetes, blood test. Loved ones and support persons are also encouraged to attend classes.

Occupational related drug and alcohol testing services are available. An Authorization for Testing form is to be completed by a company representative and sent with an employee to facilitate testing upon arrival.

Provides law enforcement services for the City of Maple Heights. Services include fingerprinting of people of all ages for identification purposes. Serves as a site for the County Children Who Witness Violence Program. Also has engraving equipment which can be used to mark identification numbers on personal property.

Provides police services for Oakwood Village. Includes community policing, where assigned officers cover the area by motorcycle, maintaining contact with residents and checking for any problems. Provides fingerprinting for employment purposes (onk only). Accepts crime reports. Records information on missing persons, follows up on them weekly, and pursues further if needed. Maintains and provides access to police reports on cases which are not still active or pending in the courts. Upon request, conducts preliminary testing of unknown substances using a field testing kit. Questionable substances will then be further analyzed by the State Bureau of Criminal Investigation (BCI).

Provides police protection services, a bicycle patrol, fingerprinting, will loan engraving equipment for property identification, and analyze unknown substances.
