Individual Support Coordinators (ISCs) support clients through coordination of services including social, medical, education, and employment. Services may include: *Identify the person’s strengths and resources. *Determine the needs of the person and work to find resources to support those needs *Create a plan that helps individual reach their goals *Connect individual and family with resources and help coordinate between multiple agencies when needed *Check in regularly to discuss updates, changes, and successes *Family supports are also available.

Provides a variety of education programs through public and parochial schools; programs include special education, tobacco, drug and alcohol prevention, before and after school and summer enrichment and extended learning, youth employment services, and preschool services.

Offers periodic Infant-toddler developmental screenings for children age 0-2 who are suspected of having a problem in hearing, language, learning, social-emotional, or motor skills. Screening takes about one hour.

Offers intensive behavioral services for children with autism. Tailored instruction focuses on communication, behavior, play, socialization, and developmentally appropriate skill acquistion.

Provides early childhood development programs to children ages birth to five from income eligible (low-income) families. Children attend early childhood centers as well as learn about computers, art, science, music, and more. Home-based services are also available as an alternative to center-based services.

Early intervention (EI) provides developmental assessments.

Provides the point of access to early childhood services (birth-3) including, referrals to ongoing Help Me Grow services such as newborn (up to 6 months) home visiting and early intervention services for infants and toddlers diagnosed with, or at risk of having, a disability or developmental delay. Provides testing for genetic or metabolic disorders, such as Byler's Disease, Cohen Syndrome, Prolidase Deficiency, PKU, etc. Prepares and coordinates a treatment plan for the patient, which can involve multiple agencies and hospitals. Special attention is paid to the Amish community, who account for a larger proportion of the rare genetic and metabolic diseases.

Offers comprehensive diagnostic services and intensive behavioral treatment for children with autism. Service includes developing a individual plan that uses a variety of behavioral techniques to increase appropriate behaviors and to decrease or eliminate interfering behaviors. Parental involvement for children of any age, but particularly for infants and toddlers, is a integral part of the treatment.
