Habilitation program serving the needs of older adults with developmental disabilities who no longer desire to work in a workshop setting. Addresses the needs of individuals who cannot work due to pronounced physical needs through occupational, physical, water, and sensory therapies.

Individual Support Coordinators (ISCs) support clients through coordination of services including social, medical, education, and employment. Services may include: *Identify the person’s strengths and resources. *Determine the needs of the person and work to find resources to support those needs *Create a plan that helps individual reach their goals *Connect individual and family with resources and help coordinate between multiple agencies when needed *Check in regularly to discuss updates, changes, and successes *Family supports are also available.

Early intervention (EI) is designed to stimulate development early in life to provide children the opportunity to maximize their abilities. Early Intervention serves children from birth to three years of age who have been evaluated and found eligible for services. A Service Coordinator assisst families with linkages to specialized services.
Provide Pack N Play portable cribs and Safe Sleep Survival Kits to families and caregivers that are unable to provide a safe sleep environment due to financial hardship. Survival Kit contains a sheet for the Pack N Play, a sleep sack for the infant, a bedtime storybook and educational materials for the parents. Family must come to the Board of Health office to pick up Pack N Play and receive instruction on proper assembly and use before taking Pack N Play home.

They ensure that families receive the most comprehensive care by: - Navigating families through the Nationwide Children's system (scheduling, follow-up visits, referrals, etc.) - Keeping families and health care providers informed of new developments in funding and advocacy - Providing community education and outreach.
Early intervention (EI) provides developmental assessments.

Medical Expense Assistance

Providing classroom programming for children 18 months to kindergarten entry with identified areas of developmental concerns. Classes run September- May with children attending 2 or 4 3-hour sessions weekly. We offer a maximum group size of 8 with the average staff child ratios of 1:4 to allow for individualized client support. We partner up with are Early Intervention Programs, school district special education staff, pediatricians, and outpatient therapy services to wrap students in the highest level of developmental support. Utilizing a play based appraoch, our goal is to prepare the child to be successful in a K-12 educational setting.

Help Me Grow Home Visiting is Ohio's program for expectant parents, newborns, infants and toddlers. The program provides health and developmental services so that children start school healthy and ready to learn. Help Me Grow works with parents to support young children's development. Home Visitors provide on-going home visits in the home; focus on improving prenatal health habits and infant care giving skills; provide information on child's health, growth, education and development through the Parents As Teachers program; provide screenings for health, hearing, vision and development; connect families with community resources and services. Affiliated with the evidence-based home visiting model Parents As Teachers. Services include - Developmental Screening - Information on parenting tips - Assistance in developing and reaching family goals - Home Visits to help each child stay "on the road" to healthy growth and development - Service Coordination - professional nurses, teachers, or other qualified staff who share their knowledge about available resources, assist families in connecting with needed services, coordinate services to help families meet their needs and goals.
