Avita Home Health & Hospice is a nonprofit organization dedicated to providing home care and hospice services, including wound care and palliative care, all in the comfort of our patients’ own homes.

When curing or stopping progression of an illness is no longer a reasonable goal, Hospice provides supportive services, nursing, social worker, home health aids, bereavement counseling, medical equipment, medicine, homemaking, pain and symptom management, spiritual care regardless of ability to pay.

Through the American Pride program at Ohio's Hospice, Veterans receive benefits and support that acknowledges their service and sacrifice. By recognizing the unique needs of our nation's Veterans who are facing a life-limiting illness, Ohio's Hospice is able to accompany and guide Veterans and their families toward a more peaceful ending. We help to assure access to all the benefits to which a Veteran is eligible, provide spiritual support and address individual post-traumatic stress issues. Through participation in Honor Flight, Veteran pinning ceremonies and special observances and celebrations, we celebrate the lives of Veterans and honor their service. On the Ohio's Hospice American Pride website, community members can enter their own story or stories about loved ones and upload photos of Veterans to honor their lives and service. The stories are searchable by name on the website and on the moving commemoration kiosk at the Ohio's Hospice of Dayton Hospice House.

Provides palliative pain and symptom management, care, and emotional support for persons with a life-limiting illness (life-threatening or terminally ill) and their families. Provides supportive services such as bereavement counseling, legal assistance, social work, art and music therapy, and spiritual care. Offers specialized AIDS and pediatric care teams. Respite care is provided for client's families.

Hospice of Dayton has a long-standing collaborative relationship with more than one hundred area nursing homes, assisted living facilities, extended care facilities and area hospitals so patients can receive hospice care in the setting where they feel most at home, whether that is in their own residence, our 51 private-bed room at our Care Center on Wilmington Avenue; our 13 private-bed in-patient facility (near the Atrium Medical Center serving Butler and Warren Co. residents); or with one of our partner organizations.

Treatment goals focus on patient comfort when cure is no longer possible and life expectancy is measured in days, weeks or months rather than years.

In Home Hospice Care
Provides personalized hospice care to veterans and their loved ones. Offers a trained staff educated in post traumatic stress disorder (PTSD), remorse and regret related to military service, substance abuse and the unique needs of those who served in the military.
Provides end of life care in the client's home 7 days a week. Home could be your own personal home or other place of residence that you reside in. If the patient's care needs require more support than may be available at home, we can assist in seeking appropriate placement.
Uses two beds for hospice patients requiring symptom management. It is a short stay treatment for patients whose symptoms cannot be managed at home or for patients who are close to death and choose not to die at home.
