Administers federal funding that provides medical, dental, and social support services to uninsured and underinsured persons living with HIV/AIDS. Facilitates the federally mandated Cuyahoga Regional HIV Services Planning Council, which determines community HIV health care needs, service priorities, and service funding allocations. Produces a Community Resource Guide listing medical, dental, and social service resources available to those living with HIV/AIDS in Ashtabula, Cuyahoga, Geauga, Lake, Lorain, and Medina Counties.

Provides and manages recycling drop-off sites and operates recycling programs in area schools. Offers recycling and community clean-up events and hosts special events for electronic, pharmaceutical, and hazardous household waste collection, as well as used tire collection and document shredding. Investigates reports of illegal dumping, littering, and burning. Develops environmentally safe solid waste management plans to achieve Ohio's goals for waste reduction, recycling, and solid waste management in the region. Helps communities and businesses implement cost-effective and environmentally safe solid waste management practices.

Promotes collaboration among public agencies that serve children and families. Seeks to improve the lives of children and to strengthen families in the community through a collaborative system. Seeks to coordinate the services of community agencies to meet the needs of families who may require services from multiple agencies and programs. Acts as a network of support services working together to assist the family to help their child.

Plans for and coordinates VITA sites throughout Cuyahoga County, and trains volunteers to work at these sites.

Plans and coordinates a range of community-based mental health and addiction treatment services, allocates funds received from Federal, State and local sources to the appropriate agencies, and provides education and prevention services to the community.

Meets monthly to provide coordination and implementation of a system of care children, youth and their families

Operates as a public health initiative committed to reducing obesity and smoking in Lima and Allen County by promoting healthy eating, active living and a tobacco free lifestyle. Does not provide health services to individuals, but works with agencies and community stake holders to promote these goals and to provide education and best practices on these issues.

Contracts with the community network and community support services to provide mental health and substance abuse services to Greene County residents.

Organizes a partnership of nonprofit, faith based and government agencies working together to assist adult felony ex-offenders, reduce the rate of recidivism, learn evidence-based practices, increase community safety, help reunite families, and make the local economy stronger.

Accept old, expired, used prescription and over the counter medications. Does not accept needles, syringes, liquids or inhalers.

Promotes the health and well-being of the community through a continuum of mental health services. Responsible for planning, funding, and monitoring mental health services in Lorain County. Funds mental health agencies so that Lorain County residents can access mental health services regardless of ability to pay. Note: Does not provide direct services to consumers.

Provides a coalition of community members, and agencies/ organizations with a vested interest in creating an environment that promotes healthy moms, babies and families. The coalition works to encourage better birth outcomes by reducing racial and ethnic disparities in infant mortality. We aim to reduce infant mortality through education and awareness, community engagement, and program implementation.

Serves Ohio with a comprehensive approach to alcohol and other drug and gambling addiction prevention, treatment, and recovery support services. The Department coordinates a statewide network of publicly-funded services designed to help all Ohioans in need of care related to substance use disorders and problem gambling issues.

Collaborates to bring together diverse organizations to work as a system, rather than as a fragmented set of resources; Employs an outcome-based funding model that measures performance, monitors programs’ success, and assures the system’s effectiveness as a whole; Combines innovative solutions and best practices with time-tested strategies to implement programs that quickly and stably house people in crisis.

Provides access to transportation information and resources to assist individuals and agencies with navigating local transportation services. Assistance includes: * Coordinated transportation services * Travel trainings, such as CarFit * Guidance on program eligibility * Speaking engagements for community groups * Assisting with communication between consumers and service providers * Coordinating the Richland Transportation Advisory Committee (RTAC) collaboration

Promotes collaboration among public systems that serve children and families. Seeks to improve the functioning of children and to preserve the integrity of families in the community through a collaborative system for multi-need, multi-system families. Can involve all major public and private agencies as needed. Focuses on issues such as mental health and substance abuse, abuse and neglect, juvenile justice, developmental disabilities, youth employment, health and wellness, and school readiness.

Offers a service to families with children/youth who have a lot of items on their plate, feel like they need help managing their child's care, or don't know how to get access to something for their child. We will help the family learn to advocate for themselves, run meetings with the services engaged in their child's care, and make recommendations where needed. We facilitate meetings as a team with the family and youth at the top of the list.
