Veterans must prove they own or rent the property that was damaged and show an intent to repair or rebuild. Must provide DD214 form and Declaration page from the insurance company showing the deductible they have, the amount the insurance was going to cover, and the claim number. If a widow is applying for assistance, must bring spouse’s death certificate and DD214 form.
