Comprehensive assessments are performed and referrals to services are made based on that assessment. Programs include Medicare, Medicaid, utility, and housing forms assistance when needed.

Refers individuals with aging and/or disability related issues to appropriate available services in the community to ensure that individuals' needs are fully met. Helps individuals find help for housing, transportation, in-home care, long-term care and other health and wellness services. Offers an internet-based, counselor assisted program that helps older adults and individuals with disabilities file for public benefits. Includes Healthy Start and Healthy Families, Home Energy Assistance Program (HEAP), child care subsidies, food stamps, voter registration and more. Provides benefit navigation, assistance with Social Security and Medicaid applications, and Social Security benefit analysis.

Provides public insurance to cover medical services that may include doctor visits, some prescription drugs, hospital care, home care, long-term care, medical equipment, transportation, mental health, vision, dental, substance abuse services and other services. NOTE: Medicaid is the payer of last resort. Medicare will be billed first if client is eligible for it.

Affordable housing units for individuals 62 and older. Units are offered at a low rate to allow comfortable, safe living for tenants who are on a fixed income.

Provides fire department and EMS ambulance transport. Will come out and check homes for carbon monoxide gas. Does not provide detectors, but will install a homeowner's unit free of charge. Administers the Senior Citizen Smoke Detector Program, installing a free, new smoke detector for seniors age 62 and older. Will also return to replace batteries and make sure all detectors are working. Non-emergency activities include fire safety demonstrations and fire code inspections of public buildings. Also provides green reflective house number signs, free of charge, to all Springfield Township/Ontario residents.

Provides a mobile app with one-one-one emotional wellness texting for older adults of the LGBTQ2+ population. Provides culturally competent, empathetic "listeners" to chat with who can provide emotional support.

Provides inhome companionship to seniors 55+ older who simply need a friend and to combat senior isolation

Offers a discount on water and sewer service to eligible seniors.

Provides a hot meal. Meal may be eaten in or taken to go.

Assists individuals with mental illness with accessing needed supports and services in order to enhance daily life skills. The goal is to improve independence in the community and reduce the impact of mental illness on daily functioning.

Service components include: assessment of need Coats for Seniors, consumer advocate, educational events and activities, friendly visitor, Golden Buckeye sign-ups, grocery shopping, home energy assistance program sign-ups, laundry service, light house cleaning, some heavy duty cleaning tasks, holiday baskets, health screenings, income tax assistance, job bank, newsletter, notary service, Ohio Energy Credits, public awareness system, recreation and socialization activities, telephone reassurance service, nutrition and medical transporation, wellness activities and programs.

Provides air conditioned and heated facility which is open to the public during extreme weather, loss of electricity, or loss of heat.

Provides an opportunity for older adults to meet and enjoy physical activities and fellowship. Activities include chair exercise, chair yoga, line dancing, bus trips, craft time, speakers, luncheons, themed parties, out to lunch gatherings, and so forth.

Arranges for in-home respite or adult day services to enable caregivers to be temporarily relieved from their care giving responsibilities.

Provides hot, nutritionally balanced meals and an opportunity to socialize with others. Offers meals on-site at some locations, and to-go at others.

Transportation provided by the Champaign Transit System to and from community resources for the purpose of applying for and receiving services, reducing isolation and promoting independent living.
