Provide services for developmentally disabled children and adults of Columbiana County. Offers an integrated continuum of education, training and family support services to eligible persons with developmental disabilities, for the entire span of life. Provide after hours emergency services.

Provides a wide range of assistance to individuals with disabilities in their own homes or apartments. Services are individually tailored to each client's needs, and may include assistance with personal care, financial management, shopping, transportation, and more depending on each indvidual's needs. Service is carried out on the basis of an annual contract, and consumer decides whether or not to renew each year.

Door to door transportation to/from services at TAC and/or other employment sites.

Provides legal assistance to parents who have children with disabilities and who are concerned about a lack of appropriate education or about (threats of) suspension and/or expulsion from elementary, middle, or high school.

Provides in-home supported living services to persons with developmental disabilities who choose to live in their own apartment or home. Amount of service ranges from a few hours a week to 24-hour care. Services include assistance with activities of daily living, money management, medication, home care, personal hygiene, meal preparation, health care and other support as needed.

Operates 20 residential facilities for adults with intellectual or other developmental disabilities (ID/DD). Offers a home-like environment and programming to help develop skills in a variety of areas including self-care, health, nutrition, and daily living techniques. Provides 24-hour supervision.

Offers one-on-one assistance to help individuals determine which private, public, state and local benefits they may be eligible to receive. Includes benefits such as Medicaid, Medicare Part D, SSI, SSD, SNAP (food stamps), HEAP (LIHEAP), Golden Buckeye Card, Exemption Prescription Assistance programs, housing and others. Will assist clients with filling out and submitting applications and forms and provides follow-up with client. Visits seniors and adults with disabilities in the home, where they provide client with combined utility assistance application, assist client, to fill it out, mail it, and follow up as needed regarding LIHEAP. Makes presentations to senior groups regarding benefit programs and provides assistance completing applications.

Provides information and referral and family support to those in the Valley affected by the autism spectrum disorder. Provides formal supervision for those awaiting trial or on community control (probation) as ordered by the court. Supervises probationers and monitors compliance to the conditions of their probation as well as providing other services such as referrals for social services. Specialized probation services provided for those needing intensive supervision such as those who are mentally disordered offenders, mentally retarded offenders, and sex offenders. Places offenders into drug treatment when needed. Provides case management for those in drug treatment and acts as a liaison between the court, client, and treatment facility. Also supervises persons in the work release and home detention programs.

Hosts a 5-day bike camp that teaches individuals ages 8 to adults with different ability levels to ride a two-wheel conventional bicycle independently.

Provides assistance to increase the energy efficiency of homes and reduce household energy expenditures. Services include: safety inspection of gas appliances, carbon monoxide alarms, education on energy saving, insulation, as well as inspection, repair, and possible replacement of furnace and hot water tank (if warranted), and customer education. Will also provide installation of water conservation devices including low-flow showerheads, toilet dams and faucet aerators. Services provided through contracted agencies. Service provided depends on the assessment of the property. Not a rehab program (home must not be vacant and utilities must be on).

Provides benefits assessments and enrollment help for older adults and persons with disabilities for more than 1,300 federal, state, and local benefits including Medicare, Medicaid, SNAP, utility help, housing help, employment assistance and more.

Offers programs designed to meet the unique behavioral health needs of individuals with developmental, behavioral, physical and mental health conditions. Also serves older adults with drug and/or alcohol addiction. Services include mental health assessment, counseling services, case management, Intensive Outpatient Program (IOP), therapy and consultation.

Provides information and support for anyone suffering form this rare eye disorder, chacterized by any abnormal blinking or eyelid tic or twitch resulting from any cause, ranging from dry eyes to Tourette's syndrome to tardive dyskinesia.

Provides a link to needed supports and to coordinate services. Assesses individual needs, facilitates development of individual service plans, arranges for services and monitors the provision of those services. Assists families with questions, planning or referrals on subjects such as Social Security, guardianship, Medicaid, waiting lists, etc.

Habilitation program serving the needs of older adults with developmental disabilities who no longer desire to work in a workshop setting. Addresses the needs of individuals who cannot work due to pronounced physical needs through occupational, physical, water, and sensory therapies.

Recreational day camp for children with and without disabilities. Children with disabilities and those without disabilities attend the same camp. Offers activities such as swimming, music, arts and crafts, horsemanship activities, and nature study.

Offers a variety of behavioral health services to address the needs of children, adolescents and their families. Services include child and adolescent psychiatry, counseling, developmental behavioral pediatrics, psychology, ADHD, feeding disorders and cerebral palsy treatment.

Provides special programming for adults with severe or profound mental retardation, developmental disabilities and multiple handicaps, as well as day programming for those with geriatric needs. Training occurs in such areas as personal hygiene and grooming, daily living skills, health care, use of leisure time and the development of prevocational skills. Our goal is to augment and maximize each person's independent functioning in all areas of their lives.

Works to help adults with developmental disabilities achieve the highest level of independent function possible. Each new client receives an individual plan which is reviewed as needed. Services include work evaluation, basic skills development, recreational activities for seniors no longer working. A number of supportive services are provided to participants, including communication skills training, recreation and activities, social work, physical therapy and occupational therapy.

Project SEARCH is a unique business led transition program for students with disabilities. It provides students who want to work a chance to explore careers and develop transferable job skills. The ultimate goal: independent adults working in a competitive, community based job. Designed as an unpaid internship program. Project SEARCH places students into real-world situations where they learn first-hand all aspects of gaining and maintaining employment. We are a partnership with OhioHealth Mansfield Hospital, Pioneer Career and Technology Center and Richland NewHope Industries.
