Provides Certified Community Health Workers who conduct intensive community service navigation, involving assisting clients to enroll in Medicaid and connecting them with medical, mental health, dental, vision care, substance abuse, and other necessary social service resources.

Coordinates care to help older adults who are in need of long term in-home health care care to remain in their home and avoid frequent hospitalizations or nursing home placement. Works with the patient, their family, and medical service providers to meet the individual needs of the patient and help them to remain as independent as possible. Agency can provide in home nursing care as well as in home health aid services, but will work with outside agencies based on the client's preference. Transportation to adult day care is provided. Dental care, vision care, respite care, rehabilitation services and hospice care can be provided by the agency as needed. If a participant needs any Durable Medical Equipment, such as a wheelchair or a lift chair, that equipment is ordered, delivered, and installed free of charge.

Plans for and implements health care events which provide free health care, dental care, vision care and educational assistance to uninsured and underinsured Ohioans. Works with a network of local agencies, free clinics, hospitals, private companies, healthcare professionals and programs in Ohio to provide care.

Provides an opportunity for residents to learn about healthy food alternatives, the benefits of physical activity, and producing fresh vegetables in their individual gardens. The National Alliance for Hispanic Health offers two individual help lines. The lines are a toll-free, reliable way to get basic health information on a variety of health issues including referral information.

Provides speakers on diseases that affect more men of certain races and ethnicities than their races or ethnicities appear in the population. Disease/condition topics include: diabetes, kidney disease, prostate cancer, high cholesterol, hypertension, and sickle cell anemia. Also offers screenings for these conditions for community events.

In 2020, the Licking County Health Department (LCHD) was one of 22 counties in Ohio to receive the Creating Healthy Communities (CHC) grant for a 5-year cycle from 2020-2024. The grant funding comes from the Centers for Disease Control and Prevention’s (CDC) Preventive Health and Health Services Block Grant which aims to prevent and reduce chronic disease throughout the state of Ohio. These funds are administrated by the Ohio Department of Health (ODH).
