Coordinates care to help older adults who are in need of long term in-home health care care to remain in their home and avoid frequent hospitalizations or nursing home placement. Works with the patient, their family, and medical service providers to meet the individual needs of the patient and help them to remain as independent as possible. Agency can provide in home nursing care as well as in home health aid services, but will work with outside agencies based on the client's preference. Transportation to adult day care is provided. Dental care, vision care, respite care, rehabilitation services and hospice care can be provided by the agency as needed. If a participant needs any Durable Medical Equipment, such as a wheelchair or a lift chair, that equipment is ordered, delivered, and installed free of charge.

Provides Certified Community Health Workers who conduct intensive community service navigation, involving assisting clients to enroll in Medicaid and connecting them with medical, mental health, dental, vision care, substance abuse, and other necessary social service resources.

Provides a health resource center open to the public. Holdings include a video resource library, printed health related materials and a database of community service contacts.

Provides general health information and provide referrals to appropriate resources.

Provides supplemental food card that includes staple foods, fruits and vegetables; nutrition education; breastfeeding education; support and counseling; and referral services to eligible women and children. Offers medical certifications to delay disconnection for those with medical conditions who require electricity to maintain their health. Individuals must request a medical certification form from AEP and have the physician complete it. Also provides third party notices.

Offers information, guidance and assistance regarding the statewide Hospital Care Assistance Program (HCAP), a program that offers free or reduced fee care at Ohio hospitals. Does not accept or process HCAP applications.
