Coordinates care to help older adults who are in need of long term in-home health care care to remain in their home and avoid frequent hospitalizations or nursing home placement. Works with the patient, their family, and medical service providers to meet the individual needs of the patient and help them to remain as independent as possible. Agency can provide in home nursing care as well as in home health aid services, but will work with outside agencies based on the client's preference. Transportation to adult day care is provided. Dental care, vision care, respite care, rehabilitation services and hospice care can be provided by the agency as needed. If a participant needs any Durable Medical Equipment, such as a wheelchair or a lift chair, that equipment is ordered, delivered, and installed free of charge.

Provides an 18 foot van designed to raise awareness of the Healthy Start Program, the program's expanded income eligibility standards, and to identify and enroll eligible children for no-cost medical care. In order to draw people to the vehicle at the event site, and to entertain children while their parents are learning about the program, Healthy Start giveaways are offered to amuse the children. These include literature, coloring books and crayons, magnets, grab bags, lanyards, sports bottles. Also offers cards with Healthy Start emergency numbers. Serves as a resource for people who are in need of services, but who do not know how to access them.

In 2020, the Licking County Health Department (LCHD) was one of 22 counties in Ohio to receive the Creating Healthy Communities (CHC) grant for a 5-year cycle from 2020-2024. The grant funding comes from the Centers for Disease Control and Prevention’s (CDC) Preventive Health and Health Services Block Grant which aims to prevent and reduce chronic disease throughout the state of Ohio. These funds are administrated by the Ohio Department of Health (ODH).

Provides 30-day emergency restoration of service for medical reasons. Customers must supply a medical certification form signed by their physician and enroll in a payment plan prior to the end of the extension. The extension may be renewed two times but cannot exceed a total of 90 days per household in a 12 month period.

A telephone-based coaching and support program for persons with chronic health condition(s) and their family and friends who care for them. Services provided include assessment to help families identify the type of help they need, steps to help them address their concerns, education, and ongoing emotional support. Those who participate receive regular check ins with staff who provide follow up on plans and suggestions for actions to take to manage the situation.
