Serves as a resource for people who are in need of services, but who do not know how to access them.

Provides a health resource center open to the public. Holdings include a video resource library, printed health related materials and a database of community service contacts.

Plans for and implements health care events which provide free health care, dental care, vision care and educational assistance to uninsured and underinsured Ohioans. Works with a network of local agencies, free clinics, hospitals, private companies, healthcare professionals and programs in Ohio to provide care.

Coordinates care to help older adults who are in need of long term in-home health care care to remain in their home and avoid frequent hospitalizations or nursing home placement. Works with the patient, their family, and medical service providers to meet the individual needs of the patient and help them to remain as independent as possible. Agency can provide in home nursing care as well as in home health aid services, but will work with outside agencies based on the client's preference. Transportation to adult day care is provided. Dental care, vision care, respite care, rehabilitation services and hospice care can be provided by the agency as needed. If a participant needs any Durable Medical Equipment, such as a wheelchair or a lift chair, that equipment is ordered, delivered, and installed free of charge.

Provides supplemental food vouchers ("nutrition cards," similar to EBT or debit cards), nutrition education, counseling, support and referral services to eligible women, children, fathers, and kinship caregivers. Offers a non-commercial health website, with information created and evaluated by Case Western Reserve University, The Ohio State University and University of Cincinnati Medical Schools. It features easy to understand information on hundreds of health topics, current health news, and allows people to ask health-related questions of a medical professional. Over 325 academic medical and research professionals from the partner universities donate their time by writing articles on health topics, and by answering visitors' questions via the site's Ask an Expert feature.

Schedules a physician to speak at a local site on any number of wellness or health-care related topics.

Offers medical certifications to delay disconnection for those with medical conditions who require electricity to maintain their health. Individuals must request a medical certification form from AEP and have the physician complete it. Also provides third party notices.

A telephone-based coaching and support program for persons with chronic health condition(s) and their family and friends who care for them. Services provided include assessment to help families identify the type of help they need, steps to help them address their concerns, education, and ongoing emotional support. Those who participate receive regular check ins with staff who provide follow up on plans and suggestions for actions to take to manage the situation.

Helps seek out options to overcome insurance denials, healthcare access issues, and medical debt crisis challenges.
