Assessment And Case Management For Long Term Care Issues | Area Agency On Aging 3

The Ohio Home Care Waiver Program (OHC) meets the needs of people who prefer to get long-term care services and supports in their home or community, rather than in an institutional setting. CareSource has contracted with the Ohio Department of Medicaid in the role of a Case Management Agency. CareSource in partnership with local Area Agency on Aging 3 will deliver case management services for the OHC program to help people meeting Medicaid financial criteria, who are under 60 years of age and assessed with an intermediate or skilled level of care, along with other program requirements.

Legal, Consumer And Public Safety | Direction Home Of Eastern Ohio

Investigates concerns and complaints about nursing homes, group homes, community based home health care, and assisted living facilities. Also handles complaints related to the MyCare managed care system. Complaints can be anonymous. Program also will help consumers select a long term care facility and provide information about local facilities. Operates a volunteer program that advocates for quality care for nursing home residents.
