Provides case planning, crisis management, emotional support, safety planning (current and future), personal advocacy, problem solving & conflict management, information & referral, scheduling and activities coordination, and other related aspects of service that a family may need. CM conducts the intake of a case and includes trauma screening as well as threat assessment. CM establishes our agency's supportive relationship with a family and coordinates all aspects of service delivery.
