Visits from a trained nurse include a physical exam for both the new mom and baby, answering questions around caring for the new baby, and provides referrals to other services and resources to support the family. Nurse visits typically occur three (3) weeks after delivery.

Provides enrollment counseling for MyCare Ohio, Ohio's Integrated Care Delivery System (ICDS) administered by the Ohio Dept of Medicaid. MyCare Ohio is a managed care plan selected by the state of Ohio to coordinate the physical, behavioral and long-term care services for individuals who are eligible for both Medicaid and Medicare. Enrollment is mandatory for people who are 18 or older, reside in Ohio and currently receive services from both Medicaid and Medicare.
