Assists with connection to family supports such as respite, camps, social activities, coaching, parent education, advocacy and assistance for paying the cost of these services. Transition planning provided between home to residential placement and back to the home and community. This includes determining the goals of placement, reviewing progress made and supports needed to bring the youth home and assure a stable environment. Structure includes a Service Coordination Steering Committee that reviews cases, approves use of funding, and monitors outcomes.
