Maintains an on-call system for timely responses to emergency medical situations by providing informed consent.

Do Not Resuscitate Or Do Not Intubate Forms | Ohio Department Of Health

Provides information about Do Not Resuscitate (DNR) orders, which relate a person's wishes not to be revived in the case of a catastrophic medical situation. A person may create a DNR order in consultation with a physician, certified nurse practitioner, clinical nurse specialist, or physician assistant. The Ohio Department of Health only provides information about the procedure's protocol. Medical questions should be discussed with a physician. Legal questions should be addressed with an attorney.

Pooled Disability Trust | Jewish Family Service Association Of Cleveland

Provides a number of services to families to enable them to make arrangements for the long-term care of disabled family members. Services include referrals to lawyers who deal with disability law as well as help drafting documents to detail individual wishes for dealing with disabled family under their care. Provides assistance with establishing and managing "pooled" financial trusts, where the assets of a number of disabled beneficiaries are pooled for purposes of investment, but separate accounts are kept for each participating individual. Also funds can only be used for supplemental services such as recreational items, according to federal law. Service includes setting up the trust, receiving, submitting, and disbursing fund requests, and providing periodic statements on individual accounts. Program is administered in conjunction with a community foundation that provides financial oversight and compliance with federal laws.
