The community navigator will work to assist individuals with substance use disorders and those that may suffer from other underlying factors that could increase the likelihood of substance use disorders with finding the necessary medical treatment. In addition to making referrals, the navigator will follow up with each individual referred to assist with ensuring they are taking advantage of the services available to them and to address barriers such as transportation.

Our navigators help pregnant people connect to prenatal care and support through home visiting programs, referrals for mental health services, and access to support for smoking cessation and other substance misuse programs. Additionally, our navigators make sure expecting and postpartum mothers have everything they need to welcome their baby home by providing access to baby items, cribs, and other material goods to meet their needs. We also provide referrals to non-baby related resources, as we recognize that the health and well being of the mother can directly impact the health of the child.
