Provides older adults and their families with services and supports needed to stay as independent as possible. Comprehensive assessments are performed and referrals to services are made based on that assessment. Programs include Medicare, Medicaid, utility, and housing forms assistance when needed.

Older Adults
Provides enrollment counseling for MyCare Ohio, Ohio's Integrated Care Delivery System (ICDS) administered by the Ohio Dept of Medicaid. MyCare Ohio is a managed care plan selected by the state of Ohio to coordinate the physical, behavioral and long-term care services for individuals who are eligible for both Medicaid and Medicare. Enrollment is mandatory for people who are 18 or older, reside in Ohio and currently receive services from both Medicaid and Medicare.
