Provides primary medical services and physical examinations to homeless veterans. Team consists of a Physician, Nurse Practitioner, Registered Nurse, and Medical Social Worker.

A registered nurse and social worker provide assistance to patients/families, including home evaluation, patient assessment, and recommendations for future care. May include, but is not limited to, help with nursing home placement, or education of the patient in current residence which might help prevent unnecessary ER visits or hospital stays. Help is also given with required financial and health care forms.

Provides social work services for those utilizing Veterans Affairs medical facilities. Social Workers are available to assist in a variety of settings to include the primary care clinic, during hospitalization, at Emergency Department visits, in mental health and rehabilitation units, and upon enrollment in specialty medical programs.
