If gender-affirming hormone treatment is part of your care plan, staff can help you coordinate your treatment with any other medications you are taking. Staff can also teach you how to give yourself injections and answer any other questions you may have.

Offers limited emergency funds to chronic kidney disease patients in need of medicine, transportation or nutritional supplements.
