Offers home visiting and early intervention programs for at-risk infants and children. Works with pregnant mothers to teach proper prenatal care so they will have a better chance of delivering a healthy child. Educates parents about immunizations, what to look for developmentally as their child grows, making a home safe for children, community linkage and how parents play a role in helping their child become ready for learning.

- Offers pregnancy care coordination services to pregnant high risk women by certified community health workers. - Works to improve birth outcomes, eliminate disparities in infant mortality, reduce low birth weight and preterm births while addressing the social determinants that affect maternal and child health in this population. - Connects clients with needed resources to reduce social deterrents such as transportation and employment, prenatal care for mothers, and safe sleep environments for babies. Services include: - Provide information on pregnancy and newborn care. - Offer problem-solving assistance to expectant mothers to resolve issues that may arise during the pregnancy. - Encourages clients to set and reach health and personal goals. - Acts as a liaison between the client and other community agencies to connect with resources and services such as medical care, housing, food, transportation, Medicaid, WIC, and other benefits.

Provides central intake to women who are pregnant or postpartum to assist in locating resources and coordinate assistance by way of Neighborhood Navigators in the client's preferred setting (home, agency and/or community setting). Performs a detailed screening to identify barriers, needs, and opportunities for women and their families and then a step-by-step approach to action is taken to each item of need. Applies for and/or enrolls individuals in various community programs that the individual qualifies for, such as WIC and Medicaid.

Offers regular, planned home visits to pregnant women and families with information on resources and skills to help them raise children who are physically, socially, and emotionally healthy and ready to learn. Offers help from health, social services and child development professionals. Home visits may include: - Supporting preventive health and prenatal practices; - Assisting mothers on how to best breastfeed and care for their babies; - Promoting parents' use of positive parenting techniques; and - Working with parents to set goals for the future, continuing their education, and find employment and childcare solutions. Program administered by Ohio Department of Health. Other participating Home Visiting provider agencies in Lorain County include Neighborhood Alliance and Ohio Guidestone.

Provides in-home support to families with newborns. Visits from a trained nurse include a physical exam for both the new mom and baby, answering questions around caring for the new baby, and provides referrals to other services and resources to support the family. Nurse visits typically occur three (3) weeks after delivery.
