The Ohio Home Care Waiver Program (OHC) meets the needs of people who prefer to get long-term care services and supports in their home or community, rather than in an institutional setting. CareSource has contracted with the Ohio Department of Medicaid in the role of a Case Management Agency. CareSource in partnership with local Area Agency on Aging, Area Agency on Aging 3 will deliver Case Management services for the OHC program to help people meeting Medicaid financial criteria, who are under 60 years of age and assessed with an Intermediate or Skilled Level of Care, along with other program requirements

Provide a higher level of personal care, shaving, bathing, skin care, catheter care, colostomy care, turning and positioning, assistance with ambulation, transfers to wheelchair, etc. Nurse aides can do vital signs and remind patient to take medication as set up by RN. Available minimum of 2 hours up to 24 hours a day by 8 hour shifts. Supervised every 2 weeks by RN. RN on call 24 hours a day. Plan of treatment submitted to physician. Also offer companion and homemaker service.

Provides skilled home nursing, home health aides (to assist with grooming, ambulation, personal care, and bathing), in-home physical, occupational, or speech therapy, and in-home respite care for eligible adults in need of these services. Also provides homemaker assistance (meal preparation, laundry and general household management including light housekeeping.) Services also include personal care and homemaking for adults with HIV/AIDS.

Provides non-medical senior homecare services including 24 hour care, hygiene assistance, light housekeeping, meal preparation, medication reminders, appointments, errands, companionship.

Works with the patient, their family, and medical service providers to meet the individual needs of the patient and help them to remain as independent as possible. Agency can provide in home nursing care as well as in home health aid services, but will work with outside agencies based on the client's preference. Transportation to adult day care is provided. Dental care, vision care, respite care, rehabilitation services and hospice care can be provided by the agency as needed. If a participant needs any Durable Medical Equipment, such as a wheelchair or a lift chair, that equipment is ordered, delivered, and installed free of charge.

Provides comprehensive home health and personal care services including: skilled nursing services, infusion therapy, pediatric services, women's health services, medical social services, private duty services, home care aides, homemakers/companions, nutritional counseling, oncology services and wound care.

PASSPORT offers home health aide services.

Program provides in-home alternatives to nursing home care for eligible individuals. Services include: a personal care plan to meet the individual's needs at home by arranging an appropriate mix of services to supplement care provided by family members and friends. Eligible services include: adaptive and assistive equipment, assistance in the home with health activities such as taking medications appropriately and other health-related activities, with case/care management, adult day care, chore service, home medical equipment and supplies, emergency response systems, home delivered meals, homemaker services, independent living assistance, minor home modifications, nutrition consultation, personal care services, medical and non-medical transportation. Program also administers the Pre-admission Review process that determines the level of care for persons who want to enter Medicaid-certified nursing homes and periodically offers vendors training on dealing with elderly/disabled individuals.

Provides non-medical in-home support for older adults and individuals who need assistance with daily activities. Services may include healthcare duties, light housekeeping, respite care, companion care. Participates in the Ohio Medicaid Waiver Program, PASSPORT, Senior Levy services, and Veterans programs.

Provides comprehensive, non-medical companionship and home care services for the elderly.
