Provides medical care by a physician in the home for one-time or ongoing medical conditions. Services can include primary care services and treatment for chronic conditions, one time assessments (including geriatric assessments), assistance developing a plan of care, help arranging other needed mobile health services, This is a non-emergency service.

Provides most of the same services patients would receive in a doctor's office but in the patient's home setting. Services include physical examinations and treatment, medication management, laboratory testing, mobile x-ray, cardiology and pulmonology consultation, transitional care expertise, and home care coordination.

Also provides Homemakers and/or Home Health Aide who are trained persons who can come to a Veteran's home and help the Veteran take care of themselves and their daily activities. Homemakers and Home Health Aides are not nurses, but they are supervised by a registered nurse who will help assess the Veteran's daily living needs.

Physicians are specialists in geriatric or internal medicine. For continuity of care, the same doctor visits the patient each time help is needed.
Provides programs that assist migrant workers in developing the knowledge and skills needed to enable them to make decisions regarding their job conditions, compensation and human rights; also provides assistance in meeting social and economic challenges and supporting the rights of the migrant worker; a mobile medical van provides basic medical services to migrant farm workers; provides immigration counseling, referrals to housing, transportation and food needs to immigrant populations.
