Offers a therapeutically oriented, ambulatory day program to military veterans only. Provides health maintenance and rehabilitative services to frail individuals in a congregate setting. Lunch is provided.

Adult Out of Home Respite Care
Program provides in-home alternatives to nursing home care for eligible individuals. Services include: a personal care plan to meet the individual's needs at home by arranging an appropriate mix of services to supplement care provided by family members and friends. Eligible services include: adaptive and assistive equipment, assistance in the home with health activities such as taking medications appropriately and other health-related activities, with case/care management, adult day care, chore service, home medical equipment and supplies, emergency response systems, home delivered meals, homemaker services, independent living assistance, minor home modifications, nutrition consultation, personal care services, medical and non-medical transportation. Program also administers the Pre-admission Review process that determines the level of care for persons who want to enter Medicaid-certified nursing homes and periodically offers vendors training on dealing with elderly/disabled individuals.

Provides a coordinated program of professional and compassionate services for adults in a community-based group setting. Off-Site examples to include but not limited to: Fishing, Ball games, Putt-Putt, Bowling, Walking, Outdoor games, Dining out, Picnics, Museums, Boat rides, Gardening, and many more. Larger activities can include but not limited to, COSI, the Columbus Zoo, Columbus Clippers, the Wilds, Theater productions, Dayton Airforce Museum & many more. Provides many free to low cost off site activities and all transportation.

Offers adult day care program. Provides one meal and two snacks each day for participants. Programming includes current event discussions, home skills instruction, educational speakers, and entertainment such as games, gardening, craft or art projects. Has an on-site nurse to help with medication distribution.
Regularly scheduled service delivered at an adult day center (center) in a non-institutional, community-based setting. Adult Day Service includes recreational and educational programming to support a consumer's health and independence goals; at least one meal, but no more than two meals per day; and, sometimes, health status monitoring, skilled therapy services, and transportation to and from the center.
Provide peer socialization and activities to adults. Yoga, physical exercises, recreational activities, games, etc. STNA on staff provides blood pressure checks as needed. Breakfast and lunch provided. Bus passes available for those who need them. A smoke free and drug free environment.
